Many diets have been proposed for the management of Crohn's disease, and many do improve symptoms, but none have been proven to cure the disease. [30] The specific carbohydrate diet usually requires adjustments by patients; if a patient finds that certain foods increase or decrease symptoms, they may adjust their diet accordingly. A food diary is recommended to see what positive or negative effects particular foods have. A low residue diet may be used to reduce the volume of stools excreted daily. People with lactose intolerance due to small bowel disease may benefit from avoiding lactose -containing foods. Patients who cannot eat may be given total parenteral nutrition (TPN), a source of vitamins and nutrients.

The GTI will give investigators the first sensitive tool to use in randomized trials to measure whether a reduction in steroids leads to less steroid toxicity. Right now, they are beginning to use the GTI in several clinical trials. To help clinical investigators and eventually physicians in clinical practice, the GTI team is looking to develop an app so that the instrument is accessible on a mobile platform. They are also developing a pediatric version of the GTI, since steroid side effects are different in young patients. A future app may allow patients to report their own adverse events.

Treatment for withdrawal from any illegal drug generally tries to offer some ways to address the impetus that originally led to the choice as part of relapse prevention. In some cases with anabolic steroids, the goal will be success in competition, as a bodybuilder, weight lifter, or other athlete. But a report from Harvard University published in the Harvard Gazette in February, 2005, cites concerning a study of body image found that to achieve a physique that they found ideal, increasing numbers of Western men were employing anabolic steroids.

Corticosteroid refractory graft-versus-host disease (GVHD) is one of the major challenges in the management of allogeneic stem cell transplant recipients. Although numerous agents have been employed to treat this patient population, no standardized second-line therapy exists. In this study, we report our experience with the administration of tocilizumab, an anti-interleukin 6 receptor antibody, in the treatment of steroid refractory GVHD. Tocilizumab was administered to 8 patients with refractory acute (n = 6) or chronic GVHD (cGVHD) (n = 2) once every 3 to 4 weeks. The majority of patients with acute GVHD (aGVHD) had grade IV organ involvement of the skin or gastrointestinal tract, whereas both patients with cGVHD had long-standing severe skin sclerosis at the time of treatment. There were no allergic or infusion-related adverse events. Treatment was discontinued in one patient over concerns that tocilizumab may have worsened preexisting hyperbilirubinemia. Several patients also had transient elevations in serum transaminase values. Infections were the primary adverse events associated with tocilizumab administration. Four patients (67%) with aGVHD had either partial or complete responses apparent within the first 56 days of therapy. One patient with cGVHD had a significant response to therapy, whereas the second had stabilization of disease that allowed for a modest reduction in immune suppressive medications. These results indicate that tocilizumab has activity in the treatment of steroid refractory GVHD and warrants further investigation as a therapeutic option for this disorder.
